Operations Readiness Review & Audit Tool (ORRAT)

Introduction
The ORA
Solution interface also hosts an integrated Review & Audit Tool (ORRAT), an additional tool that is used to conduct and manage an ORA Review or Pre-Start-Up Audit (PSUA).

Whilst the functionality of the Review and Audit tool imposes a robust and rigorous process, it also provides a range of functions, displays and reports which significantly improve the efficiency of a review or audit, whilst also reducing the effort (time, manpower and cost) required) to perform that review or audit.

For either type of review or audit, the system functionality is the same, but the groups, subjects and questions are different. The tool can be used for any type of review or audit by creating an appropriate set of questions (as a reusable template).

Terms of Reference (ToR)
Typically, a review or audit will be set up to review or audit a 'representative sample' of data from a project against a set of written terms and conditions in order to arrive at an outcome which is expressed as an 'opinion'.

Unfortunately, because this process can be somewhat variable in quality and stringency, particularly in the subject data, this leads to inconsistency in that opinion.

The ORA Review & Audit tool formalises this by allowing the Audit Leader to set these parameters from the outset, however, to maintain the necessary robustness, these cannot be modified after they are set and saved. The initial Review/Audit setup screen is shown above..

Risk Assessment Matrix (RAM)
Most companies use a standard Risk Assessment Matrix (RAM) to determine the risk posed by a particular issue or finding, quantified in terms of Likelihood, Severity and Consequences.

Although these matrices are usually set within the business processes of a particular company, they do vary across the various industries.

To enable the use the ORA Review & Audit tool fpr any industry, the Audit Leader can build a bespoke RAM at the outset of a review or audit which matches that used by the specific 'auditee' company or to prepare a tailored or bespoke version for a particular project.

Audit/Review Process Structure
Once the Audit Leader completes the setup, adding the audit team, the ORRAT process is enabled and visible to all audit/review team members and this allows the user to navigate through the structure of the Audit/Review Tool, using the expanding menu system common to the ORA Solution, to select the required Phase, Group, Subject and Audit Question as shown in the adjacent illustration.

Typically in PSUA 'mode' the system includes some 2,500 questions and in ORA Review 'mode', approximately 1,500. More focussed or pertinent questions can also be added during the actual audit or review to sharpen the focus on a particular issue.

Recording Review / Audit Findings
Review and Audit Findings are recorded against the questions listed in the Review / Audit Tool ORRAT, however, additional questions can also be added if an issue is identified and an appropriate question is not available.

Collection of Evidence
The Audit/Review Tool also includes the ORA
CARE mobile app (CARE - Collection of Audit and Review Evidence) which allows audit team members to upload photographic, video and/or audio evidence to the audit/review tool and assign it to the relevant finding.

The mobile app will also record the GPS co-ordinates of the device used to record such evidence, both to validate that evidence and to allow a precise location to be provided to the team responsible for implementing the relevant mitigating action. Evidence can also be uploaded directly into the ORRAT Tool.

CARE App (Android)

The Audit / Review status widget which is shown here in the top right corner of the Review / Audit screen is visible only to the Audit Review Leader and indicates the current status of findings entered and what effect this has on the outcome of the Audit/Review. (This prevents auditors from second guessing the outcome and prevents uncontrolled mediation of any findings).

ORRAT Findings Charts

Findings recorded in ORRAT are displayed on the above screen. Using this with the Decision Matrix shown on the same page allows users to manually determine the indicated outcome of the Review / Audit.

The Review / Audit outcome shown by the ORRAT Tool is determined using the standard findings matrix explained on the review / audit setup screen and will be expressed as one of the following opinions:

- GOOD

- FAIR

- UNSATISFACTORY

- UNACCEPTABLE

Reports Screen
The Review / Audit Process 'Reports' screen provides the Review / Audit Team Leader with the necessary additional functionality to manage, moderate and group the findings recorded by the Audit /Review team to ensure a balanced and considered opinion is arrived at.

Finally, the ORRAT System will generate a fully detailed PowerPoint presentation for the Review / Audit Team Leader to use in the close-out meeting, saving a great deal of time and effort.
